How Do I Make an Appointment?
Our offices are open from 9:00 a.m. to 5:00 p.m., Monday through Friday. The hours for Dr. Fareeha Khan will vary, but your choice of appointment times will be given every possible consideration.
While we try to keep waiting time to the minimum, emergencies may occasionally disrupt our schedule. If a significant delay in the doctor’s schedule occurs, the receptionist will try to notify you so that you will have the option of rescheduling your appointment or waiting until the doctor is available. If you are unable to keep a scheduled appointment, please notify us at least 24 hours in advance so that we may accommodate other patients who may need care.
If you are more than 15 minutes late for a scheduled appointment, it may be necessary to reschedule you so as not to inconvenience the other patients.
If you are having a medical emergency, please dial 911 immediately.
The Emergency Room
Dr. Fareeha Khan, our cardiologist in Fairfax and Tysons Corner, is available on a rotating basis with other cardiologists 24 hours a day, seven days a week. Dr. Fareeha Khan has privileges to practice at Fairfax Inova Hospital and Washington Hospital Center. While these hospitals may provide “on-call” cardiologists, you may request that she be called for any of your cardiology emergencies. The emergency room staff will make every effort to contact her or the cardiologist on call. To contact us for emergencies after hours or on holidays, please call the office telephone number and your call will be transferred to the answering service. The answering service will page the covering physician.

Do I Need a Referral?
Depending upon the requirements of your insurance plan, a referral from your primary care physician may be necessary to see our physicians. If a referral is required, it must be presented before you will be able to see the physician. This is a requirement established by your insurance plan that we must follow to be reimbursed for the services our physicians deliver.
We encourage all patients to develop a continuing relationship with a “family physician.” We will gladly coordinate your care with that physician and provide him/her with timely reports concerning your progress.
During your first visit, Dr. Khan, our heart specialist in Fairfax and Tysons Corner, will assess your condition and discuss your symptoms and medical history with you. Dr. Khan may then order a variety of tests to further aid in diagnosing your particular condition.

What Forms Do I Need to Fill Out For My Visit?
If this is your first time visiting our office, even if Dr. Khan has seen you in the hospital, you need to fill out a Patient Registration form, a Consent form, Notice of Privacy Practice Acknowledgement form, and a Medical Information form. You can obtain these forms from the website by clicking here, printing the forms on your computer, and completing them before you come into the office.
If you would rather have the forms mailed to you, please call our office and we will be more than happy to send you the forms. If neither of these is convenient for you, we suggest you come in 15 minutes prior to your appointment to fill out your paperwork.
What Hospitals Are You Affiliated With?
Dr. Khan has privileges at two hospital locations. Fairfax Inova Hospital located off Gallows Road and Washington Hospital Center in Washington, DC.
Is There Anything I Need to Do Before Coming to My First Appointment?
At the time of your first visit with our vein specialist in Fairfax and Tysons Corner, please bring your health insurance information, health insurance co-pay, referrals, list of medications you are currently taking with dosage, list of drug allergies, and other information that might be helpful in diagnosing or treating your problem (such as EKG’s, labs, office notes). Plan to arrive at least 15 minutes prior to your scheduled appointment to complete the necessary forms.
Unscheduled Office Visits
If you have a sudden problem or an acute injury, please let our staff know and we will do our best to work you into our schedule in a way that least disrupts the normal office flow, yet provides the best care for you and our other patients.
How Do I Refill My Medication?
For your convenience, ask to have your prescriptions refilled at the time of your office visit. When you have a refill on your prescriptions, call your pharmacy and have them fax a refill request to us at (703) 560-1076. If you call the clinic during business hours, you will be asked to leave the refill request with one of our staff members. Allow 48 hours for prescriptions to be refilled. If prior authorization is required, please allow 72 hours. Mail order prescriptions can either be mailed directly to you, or picked up at our office.
